4. Food Vendor Application

   World Fest is an annual event celebrating diversity through food, music, art, and friendship. Blaine World Fest is a tasting event promoting ethnic food and culture. Food vendors are not charged a fee. In lieu of a fee, it is required that food vendors have 2 - 3 items that are smaller portion and under $5 in addition to other items you would like to sell. Submittal of an application does not guarantee acceptance. Food vendors will be chosen on the following criteria: past positive participation in event, type of food (not competing with other vendors), pricing of food, and completeness of application. Food vendors will be notified via email of their status.

30. Licenses/Permits
    This event is held in Anoka County. Anoka County honors MN Department of Agriculture permits ONLY. All other food vendors will need to acquire an Anoka County Mobile Food Unit/ Temporary Food Stand/ Food Cart Temporary License Application available directly through the Anoka County Department of Health. This permit allows the food vendor to operate in Anoka County for a total of 21 days for $90.

36. Certificate of Insurance
    The liability insurance coverage shall name the City of Blaine as an additional insured, and shall have minimum limits of liability with respect to bodily injury of $100,000 for each person and $300,000 for each occurrence, $50,000 with respect to property damage. Certificate of Insurance must be received a minimum of 2 weeks prior to the event.
38. Waiver*

    I have read and agree to abide by all City of Blaine event policies outlined. I agree that the City of Blaine is not liable for any injury, theft or damage to either the buyer or seller, or their property, arising out of or pertaining to preparation for or participation in the event whether such injury, theft or damage occurs prior, during, or after the event. Vendor further agrees to indemnify and hold the City of Blaine harmless for and against any claims for such injury, theft or damage. I understand that I must carry my own general liability insurance because the City of Blaine does not provide this coverage and I understand I must show proof of such insurance with registration materials covering the event date. The City of Blaine periodically takes pictures of participants during programs and in the parks. Please be aware that these photos may be used in the City’s brochures, pamphlets or cable presentations. If you, your family members or employees do not want to be photographed or published you must give us written notice. By checking the box above, you are agreeing to the statement below.
